Terms and Conditions
VIP Membership Programs at Miller Plastic Surgery
- Your membership protects you from any price increases for the contract year.
- Membership pricing promotions and offers are non-transferable.
- Membership discounts are only valid for the VIP member’s personal use.
- Membership discounts cannot be combined with any other specials, coupons, promotions, packages or already published discounted prices.
- The ONLY two discounts that can be combined with the membership program discounts are Brilliant Distinctions (Allergen products) or Aspire Awards (Galderma products).
- 30 minute late arrival will result in a no-show.
- Appointment no-shows and same day cancellations will be deducted for the full service
- Membership Benefits carry no cash value and are non-refundable. Memberships are non-transferable.
- Diamond membership product and treatment distribution is based on 90-day calendar quarters or 4(four) 3-month periods. Products and treatments cannot be used or picked up early. If product/treatments are not picked up in their required 90-day time frame, they will be forfeited.
- Unused product or treatments cannot be gifted, and they do not carry over into the next quarter or 90-day period.
- Membership benefits cannot be transferred or shared.
- VIP member cannot split products or services, nor buy products or services for others at the member's discounted rate.
- VIP memberships are 12-month contracts.
- Monthly auto drafts from a checking account are required. You can decide to have the auto draft one the 1st or the 15th of each month.
- After your 12 month contract, your membership renews automatically and rolls into a month to month contract. In order to cancel at that time or, we require a 30 day written notice.
- A one time administrative fee of $49 is required to set up each membership.
